Nitric oxide reduction in dogs with heart failure is associated with an increase in left ventricular inotropy.
Nitric oxide (NO) is important for many biological processes, such as regulation of blood pressure, endothelium-dependent relaxation and neurotransmission. Eight dogs with naturally occurring heart failure induced by rapid ventricular pacing were compared with five healthy dogs. In the heart failure group, the effects of the NO synthase (NOS) inhibitor, NG-nitro-l-arginine methyl ester (l-NAME), on regional and global LV systolic function were assessed using ultrasonic crystals. Measurements were made before and after l-NAME. Systolic blood pressure was significantly lower in the heart failure dogs than in the healthy dogs. l-NAME increased LV afterload by reducing systolic blood pressure in both groups, without significantly changing end-systolic stress. However, l-NAME increased early (tau) and maximal (dP/dt) LV systolic function parameters only in the healthy dogs, suggesting that impaired NO production is responsible for reduced contractility in the failing heart. l-NAME had no effect on LV diastolic function or dimensions in either group.
